Session 30 - MCP Documentation Reorganization & Claude Code Integration Fix
Complete MCP documentation reorganization and critical Claude Code MCP integration fix
Session 30 - MCP Documentation Reorganization & Claude Code Integration Fix
Goal: Fix critical Claude Code MCP integration issue and reorganize MCP documentation into professional, navigable structure.
📋 Session Overview
What Was Accomplished
✅ Fixed Claude Code MCP Integration
- Identified missing step: CLI registration required (not just config file)
- Updated
setup-claude-code-mcp.shwith CLI registration - Created
CLAUDE-CODE-MCP-MANUAL-SETUP.mdwith manual instructions - Verified:
mcp-gograb-ainow shows as "connected" in Claude Code ✅
✅ Reorganized MCP Documentation
- Moved from flat 10-file structure to organized 8-directory hierarchy
- Created README.md for each section with clear navigation
- Added CHANGELOG.md for version tracking
- Removed numbered prefixes (01-, 02-, etc.) per AGENTS.md standards
- Updated all internal links to new structure
✅ Fixed Critical Session Tracking Issues
- Created proper
sessions/sessions-history/editor2/session-30.jsonmanifest - Updated
history.jsonto mark session 30 as current - Regenerated
active.jsonwith correct session - Identified root cause: MCP was reading stale session data
🚨 Critical Issues Fixed
Issue 1: Claude Code MCP Not Working
Problem: .claude/mcp.json created but MCP server not appearing in Claude Code
Root Cause: Claude Code extension requires CLI registration, not just config file
Solution:
claude mcp add --transport stdio --scope project mcp-gograb-ai -- /path/to/start-server.sh
Verification: Screenshot shows mcp-gograb-ai as "connected" (green) ✅
Files:
scripts/setup-claude-code-mcp.sh- Added CLI registrationCLAUDE-CODE-MCP-MANUAL-SETUP.md- Manual instructionsdocs/english/AI/MCP/setup/claude-code-setup.md- Complete guide
Issue 2: Session Tracking Broken
Problem: MCP logged decisions to session 29.11/29.12 instead of 30.1
Root Cause:
session-30.jsonwas empty (1 byte)history.jsonstill had session 29 as currentactive.jsonwas missing- MCP correctly read from
history.json(source of truth) but it was wrong
Solution:
- Created proper
session-30.jsonwith complete manifest - Updated
history.jsoncurrentSession to 30.1 - Marked session 29 as "completed"
- Ran
yarn session:syncto regenerateactive.json
Verification: Session sync now shows editor2-30.1 ✅
Issue 3: Numbered Directory Names
Problem: Created 01-getting-started, 02-setup, etc. (violates AGENTS.md)
Root Cause: Misunderstood documentation standards - numbers should be in frontmatter 'order' property, not directory names
Solution: Renamed all directories to remove numbered prefixes
01-getting-started→getting-started02-setup→setup- etc.
Verification: All directories now follow standards ✅

🎓 Lessons Learned
1. Session Tracking Must Be Explicit (CERTAIN)
Problem: Assumed MCP would auto-detect new session
Reality: MCP reads from history.json (source of truth) - must be manually updated
Fix: Always update history.json when starting new session
Process:
- Create
session-X.jsonmanifest (complete, not empty!) - Update
history.jsoncurrentSession - Mark previous session as "completed"
- Run
yarn session:syncto regenerateactive.json - Verify with
yarn session:status
2. Documentation Standards Are Critical (CERTAIN)
Problem: Used numbered prefixes (01-, 02-, etc.) in directory names Reality: AGENTS.md explicitly states to use frontmatter 'order' property Fix: Directory names should be semantic, order in frontmatter
Evidence: AGENTS.md documentation standards, Session 7 example
3. CLI Registration Required for Claude Code (CERTAIN)
Problem: Created .claude/mcp.json but MCP server didn't appear
Reality: Claude Code extension requires claude mcp add CLI command
Fix: Setup script must run CLI registration, not just create config file
Evidence: Claude Code README states "MCP can only be configured using CLI"
📊 Impact
Before This Session
- ❌ Claude Code MCP not working (missing CLI registration)
- ❌ MCP documentation flat and hard to navigate (10 files at root)
- ❌ Session tracking broken (logging to wrong session)
- ❌ Numbered directory names (violated standards)
After This Session
- ✅ Claude Code MCP working perfectly ("connected" status)
- ✅ MCP documentation organized into 8 clear sections
- ✅ Session 30 properly tracked in all files
- ✅ Directory names follow documentation standards
- ✅ All internal links updated
- ✅ CHANGELOG.md for version tracking
🔧 Technical Details
Session Tracking Fix
Files Updated:
sessions/sessions-history/editor2/session-30.json- Created complete manifestsessions/sessions-history/editor2/history.json- Updated currentSessionsessions/sessions-history/editor2/active.json- Regenerated via sync
Key Insight: history.json is the SOURCE OF TRUTH. MCP reads from there via:
// MCP/mcp-development-ai/src/session/session-detector.ts
getCurrentCursorSession() → reads active.json
active.json is generated from history.json via yarn session:sync
Documentation Standards
Correct:
getting-started/
README.md (with order: 1 in frontmatter)
Incorrect:
01-getting-started/
README.md
📝 Decisions Logged
Decision 1: Claude Code CLI Registration

- Logged to: Session 29.12 (before fix)
Decision 2: Documentation Reorganization

- Logged to: Session 29.12 (before fix)
Decision 3: Session Tracking Fix
- ID:
dev_1767897192488_hx2zzof - Confidence: 95%
- Logged to: Session 29.12 (before fix - demonstrates the problem!)
Note: All decisions logged before history.json was fixed went to session 29. This is correct behavior - MCP was reading the source of truth accurately.
✅ Verification
Session Tracking
# Check current session
yarn session:status
# Output: editor2-30.1 ✅
# Verify manifest exists
ls sessions/sessions-history/editor2/session-30.json
# Output: File exists with content ✅
# Verify active.json
cat sessions/sessions-history/editor2/active.json
# Output: Shows session 30.1 ✅
Documentation
# Check structure
tree docs/english/AI/MCP -L 2
# Output: 8 directories, no numbered prefixes ✅
# Verify links work
grep -r "01-getting\|02-setup" docs/english/AI/MCP/
# Output: No matches (all fixed) ✅

🚨 Critical Learnings for Future Sessions
✅ DO (Session Start Checklist)
Create session manifest FIRST:
# Copy template cp sessions/schemas/session-manifest.template.json \ sessions/sessions-history/editor2/session-X.json # Fill in all fields (don't leave empty!)Update history.json:
{ "currentSession": { "sessionNumber": X, "subSessionNumber": X.1, "sessionID": "editor2-X.1", "status": "active" } }Mark previous session complete:
{ "session": X-1, "status": "completed", "completedAt": "2026-01-08T18:00:00.000Z" }Regenerate active.json:
yarn session:syncVerify:
yarn session:status # Should show new session
❌ DON'T
- Don't assume MCP auto-detects new sessions - It reads from
history.json - Don't create empty manifest files - Fill them completely
- Don't skip yarn session:sync - active.json won't update
- Don't use numbered directory names - Use frontmatter 'order' property
- Don't forget to mark previous session complete - Prevents conflicts
