77.2 Dead Code Resolution
Documentation for Sessions
Session 77.2: Dead Code Resolution
Date: January 31, 2026 Focus: Registering unconnected tool handlers Status: Completed
Overview
During Session 77's comprehensive testing, we discovered two tool handlers that were fully implemented and exported but never registered in the MCP server's tool list. This was classic "dead code" - functional code that was unreachable.
Dead Handlers Found
1. track_outcome
Location: MCP/mcp-gograb-ai/src/services/outcome-tracker.service.ts
Status Before:
- Handler function
handleTrackOutcomewas exported - Tool was NOT in the
ListToolsRequestSchemahandler - Tool was NOT in the
CallToolRequestSchemaswitch statement
What It Does:
- Tracks decision outcomes with backend trigger activation
- Supports success/failure outcome recording
- Activates learning triggers when thresholds are met
- Queues learning jobs for pattern extraction
Schema:
{
name: 'track_outcome',
description: 'Track outcome of a decision with trigger activation',
inputSchema: {
type: 'object',
properties: {
decisionId: { type: 'string', description: 'ID of the decision to track' },
outcome: { type: 'string', enum: ['success', 'failure', 'partial'] },
impact: { type: 'string', enum: ['low', 'medium', 'high'] },
notes: { type: 'string', description: 'Additional notes about the outcome' },
metrics: { type: 'object', description: 'Optional metrics (buildTime, testsPassed, etc.)' }
},
required: ['decisionId', 'outcome']
}
}
2. report_module_outcome
Location: MCP/mcp-gograb-ai/src/services/outcome-tracker.service.ts
Status Before:
- Handler function
handleReportModuleOutcomewas exported - Tool was NOT in the
ListToolsRequestSchemahandler - Tool was NOT in the
CallToolRequestSchemaswitch statement
What It Does:
- Reports outcomes specific to code generation modules
- Tracks module-specific quality metrics
- Determines success based on multiple factors:
- Was the module dropped without edits (ideal)
- How many edits were required
- Did the build succeed
- Was it deployed to production
- Was the module deleted (failure indicator)
Schema:
{
name: 'report_module_outcome',
description: 'Report outcome of a generated module',
inputSchema: {
type: 'object',
properties: {
moduleId: { type: 'string', description: 'ID of the generated module' },
outcome: { type: 'string', enum: ['success', 'failure', 'partial'] },
wasDropped: { type: 'boolean', description: 'Was the module dropped without changes' },
editCount: { type: 'number', description: 'Number of edits required' },
buildSuccess: { type: 'boolean', description: 'Did the build succeed' },
deployedToProduction: { type: 'boolean', description: 'Was it deployed to production' },
wasDeleted: { type: 'boolean', description: 'Was the module deleted' },
feedback: { type: 'string', description: 'User feedback about the module' }
},
required: ['moduleId', 'outcome']
}
}
How They Were Registered
Step 1: Added Tool Definitions
In MCP/mcp-gograb-ai/src/server.ts, added to the tools array:
// Session 77 - Previously dead handlers now registered
{
name: 'track_outcome',
description: 'Track outcome of a decision with backend trigger activation. ' +
'Use this to record whether decisions led to success or failure, ' +
'which enables the learning system to extract patterns.',
inputSchema: {
type: 'object',
properties: {
decisionId: { type: 'string', description: 'ID of the decision to track' },
outcome: { type: 'string', enum: ['success', 'failure', 'partial'] },
impact: { type: 'string', enum: ['low', 'medium', 'high'] },
notes: { type: 'string' },
metrics: { type: 'object' }
},
required: ['decisionId', 'outcome']
}
},
{
name: 'report_module_outcome',
description: 'Report outcome of a generated module. ' +
'Tracks whether code generation produced usable code, ' +
'feeding back into generator improvement.',
inputSchema: {
type: 'object',
properties: {
moduleId: { type: 'string' },
outcome: { type: 'string', enum: ['success', 'failure', 'partial'] },
wasDropped: { type: 'boolean' },
editCount: { type: 'number' },
buildSuccess: { type: 'boolean' },
deployedToProduction: { type: 'boolean' },
wasDeleted: { type: 'boolean' },
feedback: { type: 'string' }
},
required: ['moduleId', 'outcome']
}
}
Step 2: Added Handler Cases
In the CallToolRequestSchema handler switch statement:
case 'track_outcome': {
const { decisionId, outcome, impact, notes, metrics } = args;
return await outcomeTracker.handleTrackOutcome({
decisionId,
outcome,
impact,
notes,
metrics
});
}
case 'report_module_outcome': {
const { moduleId, outcome, wasDropped, editCount, buildSuccess,
deployedToProduction, wasDeleted, feedback } = args;
return await outcomeTracker.handleReportModuleOutcome({
moduleId,
outcome,
wasDropped,
editCount,
buildSuccess,
deployedToProduction,
wasDeleted,
feedback
});
}
Impact on Tool Count
| Before Session 77 | After Session 77 |
|---|---|
| 76 tools | 78 tools |
New Tools Category: Outcome Tracking (2 tools)
track_outcome- General decision outcome trackingreport_module_outcome- Module-specific outcome tracking
Why This Matters
1. Closes the Learning Loop
The learning system was designed to:
- Log decisions (
log_decision) - Track outcomes (
track_outcome/report_module_outcome) - Extract patterns (
mine_patterns)
Without outcome tracking registered, step 2 was unreachable, breaking the feedback loop.
2. Enables Self-Improvement
With outcome tracking active:
- Successful decisions reinforce patterns
- Failed decisions identify anti-patterns
- Module quality scores improve generation
3. Metrics for Observability
Now agents can report:
- Build success rates
- Edit counts for generated code
- Production deployment success
- Quality trends over time
Verification
Tool Now Appears in List
# MCP tool list now includes:
track_outcome
report_module_outcome
Tests Confirm Functionality
// From workflow-learning.test.ts
describe('advanced outcome tracking', () => {
it('should track outcome with backend triggers', async () => {
// Test passes - tool is registered and functional
});
it('should track module-specific outcomes', async () => {
// Test passes - tool is registered and functional
});
});
Prevention Strategy
To prevent dead handlers in the future:
Automated Tool Inventory Check (
scripts/validate-tools.ts)- Scans all
handle*exports - Verifies each appears in server.ts tool registration
- Runs in CI pipeline
- Scans all
Test Coverage Gate
- All tools must have at least one test
- Tests verify tool appears in
ListToolsRequestSchemaresponse
Documentation Requirement
- Every tool must be documented in
MCP-GOGRAB-AI-REFERENCE.md - PR checklist includes documentation verification
- Every tool must be documented in
